Thomas Jones, MD

Dr. Jones is a board-certified family physician who cares for children, adults, and older adults. He provides preventive care, urgent visits, and long-term management of chronic conditions.

Dr. Jones studied at Brigham Young University and earned a master’s degree in microbiology from Thomas Jefferson University. He earned his medical degree from Spartan Health Sciences University and completed family medicine residency at the University of Wyoming, where he served as chief resident.

His broad approach to family medicine allows him to build lasting relationships with patients and care for their changing health needs over time. He values practical, personalized care and the opportunity to serve individuals and families throughout the Rexburg community.

Outside of medicine, Dr. Jones enjoys traveling and spending time with his family. He is also interested in cars and enjoys riding RZRs, snowmobiling, and finding great food.

Shelby Nordmann, MD

Dr. Shelby Nordmann was born and raised in rural Minnesota, where she completed her undergraduate education in biology, chemistry, and neuroscience. She spent her summers growing up working in Western Montana with family, which is where she fell in love with mountains and the communities in the West.

She spent high school and college working as a CNA in long-term care facilities, which solidified her interest in comprehensive family care. She completed her clinical training in Miami, Florida, working primarily with underserved populations.

Outside of medicine, she enjoys hiking, camping, backpacking, horseback riding, volunteering, and doing anything outdoors while spending time with friends and family. Her interests in family medicine are broad and span from the beginning of life to the end, including obstetrics, hospice and palliative care, pediatrics, and ongoing care for families in rural areas.

That full-spectrum perspective shapes the way Dr. Nordmann practices: with attention to the whole person, the patient’s family, and the realities of life in a rural community. She values continuity, shared decision-making, and being present for patients through both everyday health needs and major life transitions.

Dr. Nordmann also provides gynecologic care across the lifespan, with a particular focus on the often-overlooked transition through perimenopause and menopause. She offers evidence-based hormone replacement therapy from perimenopause through the years beyond, individualized to each patient’s symptoms, health history, risks, and goals.

Dr. Nordmann is currently taking new obstetrics, gynecology, menopause-care, and family practice patients.
